What are Fordyce granules
When we talk about granules of Fordyce (or Fordyce grains ), we refer to a particular condition that mainly affects the mucous membranes, ie the genital organs, the mouth but also the cheeks .
It is the formation of small sebaceous glands (called Fordyce granules) that are not associated with specific external causes, nor with hair follicles.
As we will see better in the paragraph dedicated to the causes of this problem, it is more than anything else a very common physiological condition which, more precisely, affects the following parts of the body:

  • The lips;
  • The penis and scrotum;
  • the vulva;
  • The mouth;
  • Le guance.

The name Fordyce granules derives from the fact that the dermatology specialist who was able to describe them for the first time in the history of medicine, was called John Addison Fordyce, a very famous doctor who described many other conditions.
So let’s see together specifically what these annoying glands are, but above all by what they are caused and what are the expected treatments.

What are the main causes of Fordyce granules
As we have already mentioned in the previous paragraph, Fordyce granules are a very common condition in the whole population. Indeed, it has been estimated that 79%of people report at least one Fordyce granule, certainly a very high percentage.
In general we can say that there are no precise causes that give rise to the Fordyce granules , they are associated with the presence of some pathological conditions. In practice, they can be considered as physiological events.
In fact, anyone can have Fordyce granules , there are no distinctions of gender, nor of age (even if they are very rare in very young children who are less than three years old). They cause specific symptoms
So when we talk about the

Fordyce granules, also known as Fordyce grains, or sebaceous protuberances, or Fordyce spots, we refer to the presence of certain glands of a sebaceous nature.
Generally these glands have a whitish or red color and do not exceed 3 millimeters in size. Fordyce granules generally do not create pain or itch, although they can be very annoying from an aesthetic point of view.
However, we have thought to give you a list of the main characteristics of Fordyce granules , so that you can easily recognize them should they appear. These include the following:

  • Particularly close granules can merge , giving rise to a larger gland which is often confused with the symptoms of sebaceous hyperplasia of the skin;
  • The granules that appear on the mouth or lips are white, almost yellow, very similar to tiny grains of rice;
  • The Fordyce granules that form on the external portion of the lips mainly affect the line that separates them from the normal skin of the face
  • The granules that appear on the genital organs (both male and female) are instead redder than those of the oral mucosa;
  • On the oral mucosa the Fordyce granules tend to form mainly on the inner area of the lips and on the tonsil area and can be as many as hundreds of granules;
  • Often it can happen that on the genitals they form right on the length of the penis and are highlighted when the skin is more elongated than usual, for example at the time of an erection.

These are undoubtedly the main characteristics of Fordyce granules, which, as we have said, do not cause itching or pain , only a certain discomfort from an aesthetic point of view that depends above all on the area in which they appear and on their size.

There are risks associated with Fordyce granules

There are no well-defined complications associated with Fordyce granules, apart from the strong aesthetic discomfort we talked about in the previous paragraph.
Often, however, you could confuse them with some sexually transmitted diseases as the symptom picture can be misleading. This is why some mistakenly think they have genital herpes .
However, even if it is a very remote possibility, it can happen that malignant neoplasms, known in the medical field as sebaceous carcinomas, may form from these granules.
It is precisely for this reason that it would be better to consult a doctor , or a dermatologist, who does a physical examination and illustrates all the possible risks.

What to do if we are affected by Fordyce granules
Most specialists in the field believe that resorting to therapeutic treatment is in vain. However, if the problem is evident and discomforting from an aesthetic point of view, you can decide to undergo one of the following therapies:

  • The Micro-Punch technique is certainly the most innovative and, at the moment, it seems to have excellent results. This procedure involves removing small portions of tissue through a tool very similar to a pen that makes holes;
  • Ablation with the aid of a CO2 laser (i.e. a carbon dioxide laser). However, this technique can leave scars on your skin;
  • Pulsed dye laser therapy which, although it allows us to obtain good results, is very expensive;
  • Photodynamic therapy that helps you reduce the problem of Fordyce granules by exploiting light;
  • Electro -drying which, like laser, removes papules but could leave scars;
  • Cryotherapy which, with the use of liquid nitrogen, literally freezes the papules;
  • The use of topical creams, such as those based on clindamycin.

Remember, however, that these granules are practically physiological manifestations of our body, and it is for this reason that doctors often advise against resorting to any therapy, also because some of these have not proved particularly effective in eliminating the problem.

What can we do to prevent this problem
As often happens for any other condition, also in this case the best way is that of prevention. To prevent Fordyce granules from developing, you can consider natural remedies, for example:

  • Turmeric oil;
  • Jojoba oil;
  • Coconut oil.

It is also important to have healthy and correct eating habits to prevent the formation of Fordyce granules , without forgetting that personal hygiene also plays a fundamental role.
In fact, if you keep the skin of your face clean with good cleansers and perform a scrub to remove the residues of dead cells, you will also prevent the formation of granules as you would significantly slow down the accumulation of sebum.
Likewise, the genitals should always be kept clean with special detergents for intimate hygiene, especially after sweating, for example at the end of a sports workout or during the hottest months.
